Due to the current structure of our healthinsurance system it is very difficult to know what the actual and health insurance carrier treatment costs are for different procedures. Here is a great website that goes over this information treatment by treatment:

I have found it extremely helpful in regards to gaining a better understanding of how my clients are treated and the costs associated with each of the treatment. Also, working in the individual health insurance market, each application is individually underwritten and the premium increases are directly related to the cost of treatment for each situation.
